Our Long Term Care Plan is people of all ages who have a long-term disability due to age, illness or injury. Our goal is to offer services in a community setting instead of nursing home placement when possible. You get member empowerment services to help with daily activities that only UnitedHealthcare Community Plan offers. You qualify based on income and health needs. Pangangalaga Habang Walang Sakit at Kapag May Sakit

Get the care you need to be at your best — or to get better if you are injured or sick. Kasama rito:

  • Choice of Doctor. Find a doctor you trust in our network. 
  • Medicines. Fill your prescriptions at local network pharmacies. 
  • Transportation. We pay for rides to and from medical visits or the pharmacy.
  • Case Manager. Someone to help set up and schedule your care,
  • Nurse Hotline. Speak with a registered nurse 24/7.

  • Respite Care. In-home help to give your caregiver a rest.

We want you to stay connected with easy and dependable access to health care services. If you qualify for the UnitedHealthcare MyHealthLine™ program, you can get a smartphone and a monthly service plan at no cost. (Or you can get a service plan for your current phone.)

Plus you get extras just for being a member of UnitedHealthcare Community Plan!

Pagpili ng Doktor

You get to choose a primary care physician (PCP) who is your main doctor. Gamitin ang tool na Maghanap ng Doktor para makita kung nasa aming network ang iyong doktor. 

Kung wala kang doktor o kung wala sa aming network ang iyong doktor, matutulungan ka naming maghanap ng bagong doktor na malapit sa iyo.

  • Paggagamot kung mayroon kang sakit o injury.

Pangangalaga sa Ngipin

Checkups and dental care are covered for members up to age 21.

  • Checkups and dental care are covered for members through age 20.
  • Emergency (members 21 and older have a $1,000/year limit)
  • Routine preventive and therapeutic services (ALTCS members 21 and older have a $1,000/year limit)

Exams and cleanings every six months help teeth and gums stay strong and healthy. If there is a dental problem that needs to be fixed, that’s covered as well.

Home Meal Delivery

Proper nutrition is essential to good health. Yet you may not be able to safely fix meals for yourself at home. If that’s you, we can arrange for additional meal services.

Your safety at home is our focus. That is why we conduct home visits for those needing extra help to stay at home.

During this visit we:

  • Understand your personal needs.
  • Provide minor home modifications, such as ramps and bathroom safety devices.
  • Set up emergency monitoring, if needed.
  • Arrange for personal care attendants.

Respite na Pangangalaga

Mayroon ka bang mga kapamilya o kaibigan na nag-aalaga sa iyo sa tahanan? Kung mayroon, hinihiling naming suportahan sila sa kanilang pagsusumikap. Kaya naman nagbibigay kami ng respite na pangangalaga upang makapagpahinga ang iyong mga caregiver. Ang respite na pangangalaga ay nag-aalok sa mga tagapag-alaga ng pansamantalang bakasyon malayo sa kanilang mga mahal sa buhay na may karamdaman o mga espesyal na pangangailangan sa pangangalaga.

Sometimes a person’s health and safety requires help from community programs.

We help members connect to community services. Here they can get information about a variety of programs, like:  

  • Food and housing assistance.
  • Community child care.

They can help members get much needed relief in many ways. 

Transportasyon

Our plan provides trips to and from medical appointments. 

Pangangalaga sa Paningin

Eye exams, frames and lenses including replacement and repair are covered for members under the age of 21. 

Mga Pagpapatingin Habang Walang Sakit

Makakatulong sa iyo ang mga pagpapatingin habang walang sakit sa iyong doktor na manatiling malusog. Maaaring matukoy kaagad ang mga problema sa kalusugan sa mga pagpapatinging ito upang magamot ang mga ito. Kabilang sa mga serbisyong pang-iwas sa sakit ang:
